1. Chennaiurology :

www.chennaiurology.com is website hosted and maintained on behalf of ASHVIN HOSPITALS AND CLINICS PRIVATE LIMITED. Chennaiurology also operates the internet resource www.chennaiurology.com (“Website”) on World Wide Web which is used to access Chennaiurology Software. Chennaiurology Software is developed focusing on HealthCare domain predominantly to bring systems and processes in Indian Healthcare that creates a platform for better overall experience for Consumer (Common Human ” patients, doctors, any-one with access to the Chennaiurology ) and the same helps to find, manage and organize information regarding doctors ,hospitals, pathology laboratories, diagnostic centers, medical labs, blood banks along with appointments, prescriptions, medical information, medical records, billing, inventory and accounting detail sets.

2. Protection of customer data by Chennaiurology :

2.1 Chennaiurology provides its services in “Software as a Service” model, and will host data andinformation. Chennaiurology will not review, share or distribute any such User data except as provided in the terms of use, or as may be required by law. Individual records of User data may be viewed or accessed only for the purpose of resolving a problem, support issues, or suspected violation of the terms of use, or as may be required by law. We would take all reasonable precautions for maintaining confidentiality of data. We are not responsible any breach of data (confidential or otherwise)

3. Payment, Fees and Taxes:

3.1 The Company shall endeavour to provide the Customer with facilities/gateways to pay the Fee through credit cards (American Express, visa and mastercard), debit cards, cash cards and internet banking.
3.2 It is understood and agreed by the User/Customer that the Services shall only commence after realization of money in the accounts of the Company incase online payment is being opted for by them.

3.3 It is understood and agreed by the User/Customer that payment mechanisms may be governed by separate/additional terms of use prescribed by the Company.

3.4 The Company reserves the right to refuse or cancel any order placed for a product/package that is listed at an incorrect price. This shall be regardless of whether the order has been confirmed and/or payment been levied via credit card. In the event the payment has been processed by the Company, the same shall be credited to your credit card account within 7-14 working days and duly notified to you by email. Once the order has been placed and in case You wish to cancel/modify the same You may do so subject to cancellation/modification charges as prescribed.

3.5 It is understood and agreed by the User/Customer that payment mechanisms may be governed by separate agreements between the third parties who provide facilities for such payment mechanism and the Company.

3.6 It is understood and agreed by the User/Customer that in no event whatsoever, the Company shall take any responsibility or liability for malfunctioning or defect in any payment procedure. Payment of the Price shall be the sole responsibility of the User/Customer.

3.7 The Company reserves the right to charge listing fees for certain listings, as well as transaction fees based on certain completed transactions using the Services through the Website or any other fee. The Company further reserves the right to alter any and all fees from time to time, without notice.

3.8 The User/Customer may be liable to pay all applicable charges, fees, duties, taxes, levies and assessments for availing the Services through the Website. Further, the Company reserves the right o change the Fees upon its sole discretion without any prior notice to the Customers/Users.

4. Refund Policy:

4.1 You may cancel a service and book a new one of equivalent amount against the payment that you might have already made. You will have to contact the hospital with your bill for the same.

4.2 The E-Health Package is valid for the [7] days from the time and date of invoice generation. After 7 days, the Customer shall not be entitled to claim for Services and the company shall have the right to forfeit the fees already paid in such a case.

4.3 Cancellation shall be acceptable only if the Customer informs the Company within 72 hours from the time of booking. 3% cancellation charges will be applicable on the total amount of invoice. The Customer can contact us through e-mail id, [email protected] in case of cancellation and refund.

4.4 The refund amount will be sent to the respective debit card/credit card/account from where payment was made and amount will not be refundable by any other mode

6. Need Of Privacy Policy

6.1 This privacy policy is published in compliance of:

Section 43A of The Information Technology Act, 2000;
Regulation 4 of the Information Technology (Reasonable Security Practices and Procedures and Sensitive Personal Information) Rules, 2011 (the “SPI Rules”); and
Regulation 3(1) of the Information Technology (Intermediaries Guidelines) Rules, 2011.

6.2 This privacy policy states the following:

8. Privacy Statements

8.1 This section applies to all Users (“Users”).

8.2 Accordingly, a condition of each User”s use of and access to the Website and to the Software provided by Chennaiurology to Users (together with the Website, the “Services”) is their acceptance of the terms of use and also involves acceptance of the terms of this privacy policy. Any User that does not agree with any provisions of the same is required to leave the Website immediately.

8.4 User can access, modify, correct and eliminate the data about him/her/it which has been collected pursuant to his/her/its decision to become a User. If User updates any of his/her/its information, Chennaiurology may keep a copy of the information which User originally provided to Chennaiurology in its archives for uses documented herein. Chennaiurology takes User”s rights seriously and encourages User to use them if User deems this to be appropriate. User may exercise these rights by emailing Chennaiurology at [email protected] -.

8.5 Chennaiurology may require the User to pay with a credit card, wire transfer, debit card or cheque. Chennaiurology will collect such User”s credit card number and/or other financial institution information such as bank account numbers and will use that information for the billing and payment processes, including but not limited to the use and disclosure of such credit card number and information to third parties as necessary to complete such billing operation. Verification of credit information, however, is accomplished solely by the User through the authentication process. User”s credit-card/debit card details are transacted upon secure sites of approved payment gateways which are digitally under encryption, thereby providing the highest possible degree of care as per current technology. User is advised, however, that internet technology is not 100% safe and User should exercise discretion on using the same.

8.6 Due to the communications standards on the Internet, when a User visits the Website, Chennaiurology automatically receive the URL of the site from which such User came and the site to which such User is going when he/she/it leaves. Chennaiurology also receives the Internet Protocol (IP) address of each User”s computer (or the proxy server a User used to access the World Wide Web), User”s computer operating system and type of web browser the User is using, email patterns, as well as the name of User”s ISP. This information is used to analyze overall trends to help Chennaiurology improve its Service. The linkage between User”s IP address and User”s personally identifiable information is not shared with third parties without such User”s permission or except when required by law. Such Users, including any moderators or administrators, are not authorized Chennaiurology representatives or agents, and their opinions or statements do not necessarily reflect those of Chennaiurology, and they are not authorized to bind Chennaiurology to any contract. 8.13 Chennaiurology does not knowingly collect personal data from children. However for maintaining the Personal Health Records (PHR) of their children, parents (Users) can add the vaccination and other health related information for the record keeping purpose.

8.14 Chennaiurology has implemented Indian legal industry standard security policies, rules and technical measures to protect the personal data that it has under its control from unauthorized access, improper use or disclosure, unauthorized modification and unlawful destruction or accidental loss.

8.15 Chennaiurology will not disclose any personal information or data to governmental institutions or authorities unless such disclosure is requisitioned under any Indian law or judicial decree or when Chennaiurology, in its sole discretion, deems it necessary in order to protect its rights or the rights of others, to prevent harm to persons or property, to fight fraud and credit risk, or to enforce or apply our terms of use.

8.16 Chennaiurology addresses any discrepancies and grievances of all Users with respect to processing of information in a time bound manner. For this purpose, Chennaiurology has designated “Anuraag Dubey” as the Grievance Officer, who will redress the grievances of the Users expeditiously within a maximum of one month from the date of receipt of grievance, and who can be reached by:

9. Provider”s Note:

9.1 This section is applicable to “Providers”

9.2 As part of the registration/application creation and submission process that is available to Providers on Website, certain information, including sensitive personal data or information is collected from the Providers.

9.3 All the statements in this privacy policy apply to all Providers, and all Providers are therefore required to read and understand the privacy statements set out herein prior to submitting any sensitive personal data or information to Chennaiurology, failing which they are required to leave this Website immediately.

9.4 Providers personally identifiable information they choose to provide on the Website is used to help the Providers describe / identify themselves. This information is exclusively owned by Chennaiurology. Chennaiurology may use such information for commercial purposes and in an aggregated or non-personally identifiable form for research, statistical analysis and business intelligence purposes, and may sell or otherwise transfer such research, statistical or intelligence data in an aggregated or non-personally identifiable form to third parties and affiliates.

9.5 Chennaiurology automatically lists information of Providers on its Website for every time a new Providerstarts using its Software. The Provider information listed on Website is displayed when Users search for doctors on Website and the Provider information listed on Website is used by Users to request for doctor appointments. Chennaiurology, on its own, does not list any Personally Sensitive Information of such Providers.

9.6 However, Chennaiurology does not hold any liability for any incorrect or incomplete information appearing on Website for such Providers.

10. User”s Note:

10.1 This section is applicable to Consumer (“Patients”)

10.2 As part of the registration/application creation and submission process that is available to Consumer on this website, certain information, including sensitive personal data or information is collected from the Users.

10.3 All the statements in this privacy policy apply to all Consumer, and all Consumer are therefore required to read and understand the privacy statements set out herein prior to submitting any sensitive personal data or information to chennaiurology, failing which they are required to leave this Website immediately.

10.4 If the Consumers have inadvertently submitted any such information to chennaiurology prior to reading the privacy statements set out herein, andthe Consumers do not agree with the manner in which such information is collected, stored or used, then the same may be modified or deletedby the Consumer by using options provided on the Website. In addition, any User can ask chennaiurology, by sending a wriiiten request by sending a letter marked to the address: chennaiurology, At- plot no. 10.7 At times, chennaiurology conducts a Consumer survey to collect information about Consumer”spreferences. These surveys are optional and if Consumers choose to respond, their responses will be kept anonymous. Similarly, chennaiurology may offer contests to qualifying Consumers/Patients in which we ask for contact and demographic information such as name, email address, mailing address or any other relevant information. None of this information is shared with third parties, except in summary form, if at all. The demographic information that chennaiurology collects in the registration process and through surveys is used to help chennaiurology improve its Services to meet the needs and preferences of Consumers. This may include targeting advertising to Consumers about our Services. 10.11 Save as set out above, chennaiurology only uses the personal data or information collected from Consumers in the manner set out in the section relating to “Registration Process” below. chennaiurology may also use such information in an aggregated or non-personally identifiable form for research, statistical analysis and business intelligence purposes, and may sell or otherwise transfer such research, statistical or intelligence data in an aggregated or non-personally identifiable forms to third parties and affiliates. If chennaiurology wishes to use any Consumer”s personal data for a new purpose, it will seek such Consumer”s permission before doing so.

10.12 To the extent necessary to provide Consumers with the services on the Website, chennaiurology may provide their Personal Information to third party contractors who work on behalf of or with chennaiurology to provide them with such services, to help chennaiurology communicate with Consumers or to maintain the Website. 12.5 Part of the functionality of the Software is assisting the Consumer to access information relating to them. chennaiurology may, therefore, retain and submit all such records to the relevant Users, or to their Providers (Doctors, Hospitals, Medical Labs, Blood Banks).

12.6 Notwithstanding the above, chennaiurology shall not be responsible for any breach of security or for any actions of any third parties that receive Consumer”s personal data or events that are beyond the reasonable control of chennaiurology including, acts of government, computer hacking, unauthorized access to computer data and storage device, computer crashes, breach of security and encryption, etc.

13. Change to privacy policy:

13.1 The Consumeracknowledge that this privacy policy is part of the terms of use and unconditionallyagree that becoming a Consumerof the Website and its Services signifies their assent to this privacy policy. Consumer”s visit and any dispute over privacy is subject to this policy and terms of use. chennaiurology may update this privacy policy at any time, with or without advance notice. In the event there are significant changes in the way chennaiurology treats Consumer”s personally identifiable information, chennaiurology will display a notice on the Website or send Consumers an email, as provided for above.
